Accepted authentication
No authentication (public route, or protected by link/signature).
(1) rate limit de falhas por IP; (2) política de senha ANTES de gastar tentativa/código; (3) modo factor: o método tem de estar ativo, gasta 1 tentativa (atômico) e Verify — falha ⇒ 401 invalid_factor; modo grant: o link basta; modo link: a conta continua SEM outro método (refeito aqui, não herdado do start) e o link basta; (4) CompleteReset (transação: desafio e liberação de uso único, senha, revoga TODAS as famílias, auditoria); (5) revoga o access de cada família; (6) e-mail de aviso.
passwordResetConfirm conclui a redefinição.
Retorno: 200 {ok, sessions_revoked}; 400 invalid_body | invalid_challenge_id | weak_password | breached_password | password_too_long{max_bytes} | factor_required | method_unavailable; 401 reset_challenge_invalid | invalid_factor{attempts_remaining}; 409 recovery_grant_unavailable; 429; 500.
