Accepted authentication

No authentication (public route, or protected by link/signature).

(1) cookie ausente → 401 missing_cookie; (2) hash não vivo → refreshNotLive decide entre janela de graça multi-tab (BUG-W-10, 200 rotation_race_grace_window), REUSO de token já rotacionado fora da graça (T-006: revoga a família inteira, audita, 401 reuse_detected) ou 401 not_found_or_expired; (3) família além do teto ABSOLUTO (90d desde o login) → revoga a família, 401 absolute_timeout; (4) idle da policy do time → revoga a família, 401 idle_timeout; (5) rotaciona (a filha herda a família) com expiração = min(agora+ocioso 7d, início+absoluto); corrida concorrente → 401 rotation_race; (6) assina o access com sid = família; (7) cookies com MaxAge do refresh = expiração da filha.

refresh rotaciona o refresh_token existente por um novo par (access + refresh opaco), dentro da família (sessão) do token apresentado.

Retorno: 200 {ok:true}; 401 com error opaco + reason; 500 opaco em falha interna (o detalhe vai pro log, não pro cliente).
