Duplica form (name "<orig> (copy)", status=draft)

Accepted authentication

sessionCookie — Sessão plena Axuz (cookie de sessão da casa). Sessão restrita é 403 enrollment_required.
bearerSession — JWT de acesso da sessão em `Authorization: Bearer`.

— POST /teams/{teamId}/forms/{formId}/duplicate.

Motivo: clonar form como ponto de partida sem re-fazer schema manualmente. Nome vira "<orig> (copy)" e slug vira "<orig>-copy" (ou com sufixo nanoid em colisão). Status reseta pra draft.

Comportamento: teamAndUser + parseFormID → repo.DuplicateForm → audit.Log("form.duplicated" com src_id) → 201.

Entradas: path {teamId}, {formId}.

Retorno: 201 Form novo, 400/401/404/500.
